There are two ways to register for the conference:

1. Preferred and easier method: To pay with a credit card via secure web site, click on registration for 2004 Mitchell Symposium on Observational Cosmology.
   
   In requesting banquet tickets, (or extra copies of the conference proceedings), be sure to check the box (as the form requests). Otherwise your order will not be recorded on the website.
   
   If you choose the P.O. option (to give a purchase order number rather than credit card number) be sure to click on the button for that option before submitting the registration (as the form requests). Otherwise your work in filling out the last part of the form will be lost, and the final page will have to be redone.
   
   You will receive a letter confirming your registration.
   
   
   
2. Register using check by mail, email, or fax:
   
   If paying by check mailed to arrive by April 7, please use the top part of the form below.
   
   Mail checks to Ms. Joani Groce, Mitchell Symposium on Observational Cosmology, Drawer H-1, Aggieland Station, College Station, Texas 77844, with a copy of this form.
   
   Alternatively, email the form to her at j-groce@uc.tamu.edu, or fax it to her at 1-979-845-2519; and then either mail the check to arrive by April 7 (early registration fee) or pay at the conference (late registration fee).
   
   If paying at the conference, please use the lower part of this form.
